Haemocoagulase from Agkistrodon acutus for patients undergoing endoscopic sinus surgery

Objective To evaluate the hemostatic effect of Haemocoagulase from Agkistrodon acutus (HCA) in nasal endoscopic surgery. Method: Sixty patients who underwent elective endoscopic sinus surgery were randomly divided into an observation group (R group) and a control group (F group), with 30 patients in each group. In group R, 2 U hemagglutinating enzyme of Agkistrodon acutus was given 15 minutes before operation, while in group F, no hemostatic drugs were used. Record the intraoperative bleeding volume of two groups and evaluate the hemostatic effect of HCA. The total effective rate in group R was 28 cases, accounting for 93.3%; Group F had a total of 21 effective cases, accounting for 70.0%. The hemostatic effect of Group R was better than that of Group F (P<0.05). Conclusion Haemocoagulase from Agkistrodon acutus has a good hemostatic effect in endoscopic sinus surgery. [Key words] Haemocoagulase of Agkistrodon acutus; Hemostasis; Nasal endoscopic surgery; Nasal endoscopic surgery is characterized by mild trauma, good surgical effect and increasingly extensive clinical application. Because the submucosal blood vessels in the front and lower part of the Nasal septum converge into the Litter area by arterial branches, and the veins form the Klinefelter venous plexus, and the endoscopic surgery has a small field of vision, it is difficult to use conventional surgical hemostasis methods, while the diseased tissues rich in blood vessels often bleed violently, which may lead to blurred surgical fields, thus causing a series of surgical complications, And it seriously affects the surgical effect [1]. Thrombin like enzyme (TLE) is a protease based hemostatic drug derived from animals. Due to its low toxicity, fast onset, and long-lasting efficacy, it has been widely used in various surgical procedures to stop bleeding in recent years. Hemagglutinin (HCA) is a kind of snake venom hemagglutinin isolated from the venom of Agkistrodon acutus [3], which is a new drug registered in the country. It is reported in literature [4-5] that the newly discovered single component hemocoagulase of Agkistrodon acutus has good hemostatic effect and higher safety. From June 2011 to February 2012, the author gave 2 U of Haemocoagulase of Agkistrodon acutus to patients undergoing endoscopic sinus surgery 15 minutes before operation, and evaluated its hemostatic effect by observing the amount of bleeding during operation.
